Hemostasis in cardiac anesthesia
A knowledge base to understand perioperative bleeding and coagulation: physiology first, evidence second, clinical decisions in context.
Guideline-based reading roadmap Outline · 20 Oct 2026Cardiac surgery can expose patients to bleeding and thrombosis risks simultaneously. Cardiopulmonary bypass, systemic anticoagulation, platelet dysfunction, fibrinolysis, hypothermia, and surgical bleeding all affect how we interpret coagulation findings and intervene. This page defines a study sequence; it is not a ready-to-use treatment algorithm.
1. A framework for learning
2. Study roadmap
- Core physiology: primary hemostasis, coagulation, thrombin generation, fibrinolysis, and clot stability.
- CPB and anticoagulation: heparin, ACT, reversal, and common interpretation pitfalls.
- Diagnostics: platelet count, fibrinogen, PT/INR, aPTT, and viscoelastic methods (TEG/ROTEM).
- Blood management: preoperative optimization, blood-conservation strategies, transfusion decision-making, and targeted hemostatic therapies.
- Evidence appraisal: recommendation strength, study populations, comparative outcomes, costs, and uncertainties.
- Cases: structured discussions of post-CPB bleeding using fictional or appropriately de-identified scenarios.
3. Questions I want to be able to answer
- How can I distinguish ongoing surgical bleeding from a coagulation problem without overinterpreting a single test?
- How does cardiopulmonary bypass change platelet function and fibrinogen availability?
- What can viscoelastic testing reliably tell us—and what can it not tell us?
- Which interventions have evidence for reducing bleeding, transfusion, or patient-important adverse outcomes?
- How do recommendations for adult cardiac surgery differ from pediatric and congenital cardiac practice?
4. Guideline reading list
The 2024 EACTS/EACTAIC patient blood management guideline, produced with the EBCP, is a core reference for adult cardiac surgery. It addresses preoperative optimization, intraoperative management, transfusion and coagulation, and postoperative care.
It should be read alongside the 2024 EACTS/EACTAIC/EBCP cardiopulmonary bypass guideline and the 2021 STS/SCA/AmSECT/SABM patient blood management update. Recommendations developed for adult patients should not simply be transplanted to children with congenital heart disease.
5. What this track can become
- A short original explanation of hemostatic changes during cardiopulmonary bypass.
- A critical review of individual guideline recommendations and their supporting evidence.
- A presentation on interpreting post-bypass bleeding investigations.
- Fictional case-based discussions, each linking back to a deeper article.
- Short LinkedIn, Instagram, and X summaries, with links to sources and limitations.
